A daily pill that slashes cholesterol

The US FDA has approved Lipfendra (enlicitide), the first once-daily oral PCSK9 inhibitor, for adults with high LDL cholesterol, including those with heterozygous familial hypercholesterolaemia. Earlier PCSK9 inhibitors were available mainly as injections.

What is ApoB?
Apolipoprotein B (ApoB) is the main structural protein present on potentially harmful cholesterol-carrying particles such as LDL, VLDL, IDL and lipoprotein(a). Since each atherogenic particle carries one ApoB molecule, the ApoB blood level estimates the total number of artery-clogging particles, rather than merely the amount of cholesterol they carry.

Why is ApoB Reduction Important?

• A high ApoB level means more harmful particles can enter artery walls and form plaque.
• Reducing ApoB lowers the risk of atherosclerosis, heart attack and stroke.
• ApoB may reveal cardiovascular risk even when LDL cholesterol appears normal.
• It is especially useful in people with diabetes, metabolic syndrome or high triglycerides.

How Does It Work?

• PCSK9 normally reduces the number of LDL receptors in the liver.
• Lipfendra blocks PCSK9, allowing more LDL receptors to remove “bad” cholesterol from the bloodstream.
• It is prescribed as an addition to diet, exercise and existing cholesterol-lowering treatment—not as their automatic replacement.

Who May Benefit Most?

• Patients at high or very high cardiovascular risk.
• People with previous heart attack, angioplasty, stent placement or established coronary artery disease.
• Patients with familial hypercholesterolaemia.
• Those whose LDL remains high despite maximally tolerated statins and other medicines.

Comparison with Existing Treatments

• Statins remain the first-line treatment because of extensive evidence that they reduce heart attacks and strokes.
• Injectable PCSK9 inhibitors are highly effective but may be inconvenient or expensive.
• Lipfendra offers similar PCSK9 inhibition in a convenient daily tablet, which may improve treatment adherence.

Does It Prevent Heart Attacks?

• Lipfendra has demonstrated strong LDL and ApoB reduction, but whether the drug itself lowers heart attacks, strokes and cardiovascular deaths is still being evaluated in an ongoing outcomes trial.

Lipfendra represents an important advance for patients needing substantial additional cholesterol reduction, but its use must be based on individual cardiovascular risk and medical advice.
